St. John Ambulance of Malaysia is organising blood donation activities to replenish the blood bank. Please TAKE NOTE OF EXTRA STEPS during this MCO period.
There are two forms to fill prior to visiting the blood donation venue.
1. Donor Registration Form

2. For appointment and travel slip

After filling in the forms, the Blood Bank will then email you a traveling slip. Please print and bring along to ease your travel to respective locations.
